- An official, signed license verification letter or form from the West Virginia Board of Optometry is available upon request by any West Virginia Optometry licensee. The cost for each single license verification is $25.00 as set forth in W. Va. State Rule §14-5.
- The cost for the full list (written or electronic) of all active licensed optometrists in West Virginia is $200.00 as set forth in W. Va. State Rule §14-5.
Payment can be made online, by check or money order made out to the West Virginia Board of Optometry and mailed to the office address. Payment must be received before a license verification or the full list of active licensed Optometrists in West Virginia can be issued.
