The West Virginia Secretary of State’s Office requires that the West Virginia Board of Optometry verify any new optometric business entity registering to do business in West Virginia. This includes any optometric corporation, limited liability corporation and professional limited liability corporation.
The West Virginia Optometric Business Entity Registration Fee is $25.00.
Please contact the Board for an application and additional registration details.
Upon completion, the Board will provide the Optometric Business Entity with a signed VOE (Verification of Eligibility) form to provide the West Virginia Secretary of State's Office. Each year that you renew your WVSOS business registration, you must file an Annual Report. Each Optometric Business Entity is supposed to provide the Board of Optometry with a copy of that Annual Report.
