- An official, signed license verification letter or form from the West Virginia Board of Optometry is available upon request in writing. The cost for each single license verification is $25.00 as set forth in W. Va. State Rule §14-5.
- The cost for the full list (written or electronic) of all active licensed optometrists in West Virginia is $200.00 as set forth in W. Va. State Rule §14-5.
Payment can be made online, by check or money order made out to the West Virginia Board of Optometry and mailed to the office address. Payment must be received before a license verification or the full list of active licensed Optometrists in West Virginia can be issued.
